How does Remote Patient Monitoring Work?

We provide you with devices to monitor your chronic conditions in the comfort of your home, and in real time. We routinely reach out to you to collect condition-specific reports and help you maintain control of your health.


How will RPM help your hypertension condition?

If you are living with hypertension, we have great news. Research has shown RPM can reduce systolic blood pressure (SBP) and diastolic blood pressure (DBP) significantly compared to usual care and self-monitoring alone.


Can RPM help your atrial fibrillation or congestive heart disease?

With remote patient monitoring, we have the potential to improve your condition and possible outcomes. Devices enable accurate and early detection, which decreases all-cause mortality rates and hospitalizations.


Can RPM Help with Diabetes?

RPM shows significant improvement in HbA1c levels. The greatest effect is seen in telephone-delivered interventions, followed by blood glucose monitoring system interventions, and lastly, interventions involving automatic transmission of SMBG using a mobile phone or a telehealth unit.
